6^th grade Off-site Permission Form

I, ______________________________________, parent/guardian of ______________________________________________ herby give my permission for the above named child to participate in the off-site mission program of the Sierra Vista UMC Vacation Bible School program.  I understand that my child will be traveling in the church van/bus driven by licensed drivers.  I further understand that all vehicles will be supervised by a non-driving adult.

I understand that the 6^th graders will be visiting various mission related sites within the city limits of San Angelo on June 9-12, 2008.  I further understand that my child will be returning to Sierra Vista UMC in time to participate in the daily closing ceremonies.
